MAINTENANCE REQUEST FORM

If you have a life threatening emergency please dial 911. If this issue is not a life threatening event please provide the following information or call 303 356-5508 and we will assist you in resolving your issue.

Emergency calls would include the following items:

•  Fire
•  Gas smell
•  Floods or substantial leaks
•  Drains that are backing up
•  Lock malfunctions
•  Toilet overflowing
•  Heating not functioning in winter
•  Major electrical problems i.e. over 50% of your space lacks electricity

For all other maintenance needs please provide the following information:

* Building and unit #

* Name of contact person

* Phone number where you can be reached

* Alternate phone number

* Email address

* Date and time of the call

* Nature of the problem – please be specific so we can respond ccordingly
